Conversations that Count

OUR OBJECTIVE

This program aims to facilitate early essential planning by fostering open communication in a safe and trusting environment. Our relational methodology will enhance the capacity of organisations, families, and individuals to proactively discuss end-of-life matters.

WHAT YOU WILL RECEIVE

Increased capacity for end-of-life discussions:

Increased confidence of care providers, families, and individuals to discuss end-of-life matters proactively and ensure their wishes are known and recorded.

Expanded communication:

Create an environment that makes it easier for individuals to have compassionate conversations with others about the social and emotional needs associated with the end of life, and provide a model for others to do the same.

Improved knowledge of service options:

Equip individuals with tools and knowledge to plan with clarity and confidence for their personal or others’ death.

Networked support:

Foster collaboration with other organisations seeking to improve the death experience to encourage development of a local peer support network.
